Looking at an Apple TV. My wife and I both lived in the States before, and now live in the UK. As such we’d like to watch US Sports (Peacock, Fox Sports, ESPN etc.), as well as US networks that stream.
My old TV was Google OS, and this was easy by sideloading relevant apps (Express VPN). I know Apple is a little restrictive on this sort of thing, so wondering if A) It’s even possible to download the apps (wife still has her American Apple ID which may help) and B) Once downloaded, we won’t be geolocked and unable to watch anything?

If you don’t want a VPN you can run with a Smart DNS service like DNS4Me, I have this running on my Apple TV’s, very easy to set up.
I’ve have a little knowledge in this area.
As you are already aware, you can’t add a VPN on an appleTV.
Here is the next issue, are you able to access you’re modem settings or are your modem settings locked and only accessible by you’re Internet provider.
If you’re able to access your modem settings you’ll be able to configure your modem with the help of a VPN. If your not able to access your modem settings yourself you can contact your internet provider and ask them if they will modify youre modem settings to all for your Apple TV to think its in the US.

Can I then download from the US App Store still. Because on my iPhone I can’t download fox sports for example?

AppleIDs are tied to country so you can by using your wife’s AppleID to download apps from the US store.
Just add her as another user on the AppleTV and just switch to her profile when you wish to use the US storefront, once apps are downloaded it then doesn’t matter which profile you’re on to use them.
They have guides for all boxes, including AppleTV
You basically change the DNS Servers in your Apple TV wifi settings.

I just checked and I see that you can now download certain vpn apps
If you have Nord then you can download the app onto your Apple TV. Not all VPN’s offer iOS apps.
