Is your vpn working with apps on the firestick?
Mine hasn’t been working for like 2 weeks (shows correct vpn IP in system checker “public IP” but IP doesn’t change on the stick, my apps are stuck in US)
I even switched vpn providers and it’s still not allowing apps to connect via vpn.
I can use a browser on my stick and it will be in the correct VPN country, but Netflix, prime, YouTube, etc don’t work with vpn country - only come up as USA, no vpn warnings, just using the local IP

I was using epxressvpn for netflix US using their dns media streamer method which worked for years but lately it doesn’t worked anymore
It happened when I made the upgrade from firestick 2017 to firestick 4k. On the old stick I never upgraded the netflix app.
Is there any place where I can find some old netflix apk which can work properly for firestick? I tried apkmirror and other sites but all I get is the mobile versions not the versions for fire os
I had do dump expressVPN too. Same thing.
Every time there was a problem I was wasting time doing all the stuff you mentioned , then contacting their support.
They would then insist I did it all over again with 1st level, before they would contact 2nd level, who would say that there are problems with service XXX.
The problems would often run intermittently for weeks. I got a refund and changed provider.

After clearing the caches on my firestick, uninstalling and reinstalling Netflix, changing the DNS on my modem (to the ones in Surfshark’s troubleshooting guides), and then changing my internet connection DNS on the (entering the IP addresses manually and changing the subnet mask).
I FINALLY GOT IT TO WORK

I’m using Surfshark now instead of ExpressVPN.
I never had any problems with Express before (2-3 years with them) other than when Netflix would catch up to their servers, but even the customer service couldn’t figure this out even being connected with me for over an hour, maybe longer. So I requested a refund from Express and tried Surfshark (which I’m not complaining yet, because it’s like a quarter of the price - Express wasn’t working for Netflix on my phone either but Surfshark loaded that up just fine from the start)
I have PIA vpn and it seems to work fine on my old fire stick. Got a 4K fire stick for another tv and PIA vpn doesn’t seem to work no matter what I do. Amazon changed something on the new 4K fire sticks that make some vpns not work I guess.

I have a 4k stick and use windscribe vpn and it works perfectly. Maybe try them? The even have a decent free trial period to try it out first.

It seems the issue I have is related with the netlfix latest app on firestick 4k. If anyone knows where to get an old app firestick compatible I will be grateful
Had the same issue, got Surfshark cheap through a deal from r/vpncoupons just for testing and worked fine, didn’t need to tweak anything, so sticking with it for now too
