Previously, the Firefox extension was working well.
Today, when I connect, I get a popup that says:
Authentication Required - Mozilla Firefox
The proxy moz-proxy://https-us-washingtondc.privateinternetaccess.com:80 is requesting a username and password. The site says: “Private Internet Access”

Why do I see this prompt from Firefox and not the extension itself? My username and password are correct inside the extension.
I initially was trying to connect to a down server (it said it was connected anyway) and then I switched to an active server.
PIA is having a problem migrating to Next Generation Network. For example, Washington DC works for me. But New York and Finland don’t.
This has likely occurred with the most recent update to the Firefox browser. To correct this, please visit the options menu within the browser.
Once in the options menu, click on the Privacy & Security tab.
From here, please scroll down until you reach the History setting and change the “Firefox will” setting to “Remember history” instead of “Never remember history”.
If this does not help, try uninstalling your extension and reinstalling it. If the issue persists, please submit a ticket to our Helpdesk labelled ‘ATTN SMR’ for assistance. You can do this using the following link - https://www.privateinternetaccess.com/helpdesk/new-ticket
It seems that every server I connect to through the Firefox extension gives this popup.

Thanks for your help, but here is what each step did and how I solved it. Hopefully this helps for future help tickets.
What didn’t work:
From here, please scroll down until you reach the History setting and change the “Firefox will” setting to “Remember history” instead of “Never remember history”.
This works temporarily. This option puts you into Private Browsing mode full time. So, every time you quit, the PIA extension forgets your username and password. You then have to enter the username and password each time you use the browser. This is incredibly frustrating.
When I revert to “Use custom sections for history” but check “clear history when Firefox closes”, the PIA extension will remember the username and password, but gives me the same frustrating popup as I initially posted about asking for the username and password.
Solution:
Fortunately, removing the PIA extension and reinstalling it seems to remember the username and password, and does not give the initial error message I was getting. I followed the link on the PIA Downloads page to reinstall the extension.
